The Wellness Practice-Global Self Health Corp. (We/Us) is dedicated to maintaining high standards of confidentiality with respect to any information that has been provided to us. This Policy Statement has been prepared to inform you of our practices concerning the collection, use, storage, and disclosure of Personal Information, General Information, and/or Clinical Information. For the purposes of this policy, "Clinic" means any individual or company who is has been authorized to access the Clinic System and "Patient" refers to any individual or company able who has been authorized to access the Patient System.

Our obligations apply to all officers, employees, contractors and agents who provide services to or on behalf of the The Wellness Practice-Global Self Health Corp. in connection with our delivery of products, services and information to our patients.

What is Personal Information?

We consider "Personal Information" to mean any information, recorded in any form, about an individual that MIGHT allow a third party to infer or determine the identity of that individual. The Personal Information that we collect is a patient's first name, last name, and email address. Date of birth and telephone number may also be collected when required by third party blood testing facilities.

What is Public Information?

We consider "Public Information" to mean any information, recorded in any form, about a Clinic that MIGHT allow a third party to infer or determine the identity of that individual. The Public Information that we collect is a clinic's business name, address, and contact information. The Clinic maintains control over their Public Information. Public Information will be made available in the Patient System for the Clinic's patients and may also be made available to the general public in the Evidence-Based Lifestyle Protocols website, or by other means of communication.

What is General Information?

We consider "General Information" to mean any information, recorded in any form, about an individual that COULD NOT be used by a third party to infer or determine the identity of that individual. The General Information that we collect is age, gender, state/province and country of residence.

What is Clinical Information?

We consider "Clinical Information" to mean any information, recorded in any form, about an individual that pertains to the Lifestyle Health Risk Assessment. Each individual component on its own is NOT ENOUGH to allow a third party to infer or determine the identity of that individual, however if enough of these items were disclosed it MIGHT allow a third party to infer or determine the identity of that individual.  The Clinical Information that we may collect is a patient's age, height, weight, body mass index (BMI), waist circumference, hip circumference, waist/hip ratio, a variety of functional health parameters such as strength, aerobic capacity, and the answers to a series of questions regarding lifestyle choices.

Why We Collect Personal, General, and/or Clinical Information

We collect information that you choose to share with us in the registration and assessment sections of our site. This information may include limited Personal information as described above as well as General and Clinical information. We use this information to create your individual reports as well as to collect general statistics regarding the results of our Lifestyle Health Risk Assessments. This information is saved to our database and remains confidential. This saved information is collected only with your permission and may be updated or deleted by us at any time. If you choose to not provide the information required for registration, you may not register with the site and you will not be able to use the site.

How We Use Personal Information

We only use Personal Information for purposes that would be considered reasonable in the circumstances and only such information as is required for the purposes of providing products, services or information to our patients.  We use only fair and lawful methods to collect Personal Information.

The use of, and access to, your Personal Information is strictly limited to identifying your account and collecting all of your information in one data file. The only individuals who have access to your Personal Information are you, the Clinic that has created the Patient account, and our database administrator.

How We Use General Information

We only use General Information for purposes that would be considered reasonable in the circumstances and only such information as is required for the purposes of providing products, services or information to our patients.  We use only fair and lawful methods to collect General Information.

The contemplated use of, and access to, your General Information includes, but is not limited to, allowing us to provide general statistics as to how many individuals of a particular age, gender and/or region are participating in our program.  This public use of your General Information would never be done is such a way as to allow your Personal Information to be accessed by unauthorized users.

How We Use Clinical Information

We only use Clinical Information for purposes that would be considered reasonable in the circumstances and only such information as is required for the purposes of providing products, services or information to our patients.  We use only fair and lawful methods to collect Clinical Information.

The contemplated use of, and access to, your Clinical Information includes, but is not limited to, allowing us to provide general statistics as to how many activities have been performed by individuals participating in our program (e.g. pounds lost, pushups performed, miles walked, vegetables eaten, etc.). This public use of your Clinical Information would never be done is such a way as to allow your Personal Information to be accessed by unauthorized users.

When We May Disclose Your Personal Information

As a general rule, all Personal Information concerning a patient is held in strict confidence and is not revealed to anyone unless expressly authorized by the patient.

We do NOT under any circumstances, other than those listed below, release the name, address, telephone number and e-mail address of ANY of our patients to ANY third parties.

Please note that there are circumstances where the use and/or disclosure of Personal Information may be justified or permitted or where we are obliged to disclose information without consent.  Such circumstances may include:

  • Where required by law or by order or requirement of a court, administrative agency or other governmental tribunal;
  • Where we believe, upon reasonable grounds, that it is necessary to protect the rights, privacy, safety or property of an identifiable person or group;
  • Where it is necessary to establish or collect fees;
  • Where it is necessary to permit us to pursue available remedies or limit any damages that we may sustain; or
  • Where the information is public.

Where obliged or permitted to disclose information without consent, we will not disclose more information than is required.

Consent

Unless permitted by law, no Personal, General and/or Clinical Information is collected, without first obtaining the consent of the individual concerned to the collection, use and dissemination of that information.

However, we may seek consent to use and disclose Personal, General, and/or Clinical Information after it has been collected in those cases where we wish to use the information for a purpose not identified in this Policy or not previously identified or for which the individual concerned has not previously consented.

If a patient consents, his/her Personal, General, and/or Clinical Information shall be used for the purposes indicated in this policy statement.

The Accuracy and Retention of Personal, General, and/or Clinical Information

We endeavor to ensure that any Personal, General and/or Clinical Information provided by our patients and in our possession is as accurate, current and complete as necessary for the purposes for which we use that information.

We keep your Personal, General, and/or Clinical Information indefinitely. This period may extend beyond the end of a patient's relationship with us or with the Clinic that has created the Patient account. We are required to keep your Information in order to accurately report the results of our program.

Protection of Personal, General, and Clinical Information

We endeavor to maintain appropriate physical, procedural and technical security with respect to our offices and information storage facilities so as to prevent any loss, misuse, unauthorized access, disclosure, or modification of Personal, General and/or Clinical Information.

We further protect your Information by restricting access to it to those employees that we have determined need to know that information in order that we may provide our products, services or information.

If any officer, employee, contractor, and/or agent misuses your Information, this will be considered as a serious offence for which disciplinary action may be taken, up to and including termination of employment. If any Clinic misuses your Information this will be considered a serious issue for which action may be taken, up to and including termination of any agreement between us and that individual or organization.

We audit our procedures and security measures from time to time to ensure that they remain effective and appropriate.

Access to Your Personal Information

The use of, and access to, your Personal Information is strictly limited to identifying your account and collecting all of your information in one data file. The only individuals who have access to your Personal Information are you, the Clinic that has created the Patient account, and our database administrator.

Access to Your General Information

The use of, and access to, your General Information is limited to statistical and informational purposes (e.g. identifying how many individuals of a particular age, gender and/or region are participating in our program). The only individuals who have access to your General Information in connection with your Personal Information are you, the Clinic that has created the Patient account, and our database administrator.

Access to Your Clinical Information

The use of, and access to, your Clinical Information is limited to preparing your individual reports and for general statistical and informational purposes (e.g. how many activities have been performed by individuals participating in our program such as pounds lost, pushups performed, miles walked, vegetables eaten, etc.). The only individuals who have access to your Clinical Information in connection with your Personal Information are you, the Clinic that has created the Patient account, and our database administrator.

Visiting the Evidence-Based Lifestyle Protocols Web site

A visitor to our web site (www.wellnessandprevention.com) is not required to reveal any individually identifiable information, such as name, address, or telephone number. Nor is such information collected passively by electronic means.  In order to access and use the patient-only portions of the site, patients will need to provide certain Personal Information. The terms for the use of this information is described above.

Our web server does collect visitor information in the form of the visitor's domain or Internet Protocol (IP) address and does collect information regarding which pages are being accessed. This information is used internally, only in aggregate form, to better serve visitors by helping us to:

  • Manage our sites;
  • Diagnose any technical problems; and
  • Secure our website; and
  • Improve the content of our web site.

Following its use, the information collected is not retained but, rather, is discarded in a secure manner.   
For the transfer and receipt of certain types of sensitive information such as financial information, visitors will be re-directed to a secure server.

In browsing the Internet, you will encounter a technology called "cookies" which can be used to provide you with specific information from a Web site as well as provide the operator of a web site with information about you. "Session" cookies are temporary bits of information that are erased once you exit your Web browser window or otherwise turn your computer off. Session cookies are used to improve navigation on Web sites and to collect aggregate statistical information. This site uses session cookies to only record session information.

Most Internet browsers are initially set to accept cookies.  If you do not wish to accept cookies, you can set yours to refuse cookies or to alert you when cookies are being sent.

Evolving Practices

This statement is in effect as of February 1, 2013. We will from time to time review and revise our privacy practices and this Policy. In the event of any amendment, a notice will be posted on www.wellnessandprevention.com and published in appropriate locations on the site.  Policy changes will apply to the information collected from the date of posting to the web site as well as to existing information held by us.
